Unemployment Compensation
A claim for Unemployment Insurance may be filed in person at any Arkansas Workforce Center office.  A valid government ID is required (Driver’s License or Passport). UI claimant forms are also available online, as well as located in most public buildings for filing claims by mail. Learn more about filing a claim and eligibility for benefits. EZARC is another option to file an initial or additional claim for Arkansas Unemployment Insurance via the Internet. If you are working less than full-time, or have completely separated from your last job, you may file your Arkansas Unemployment Insurance claim from your own home computer. To file an EZARC online claim, you must have access to a computer, a printer and an Internet connection.

Vital Records
Arkansas Vital Records has birth records dating from February 1, 1914 through the present. There are a limited number of birth records available prior to 1914. Those records were filed with Arkansas Vital Records after 1914. We also have original copies of some Little Rock and Fort Smith births dating from 1881. The cost of a birth record is $12.00 for the first copy and $10.00 for each additional copy of the same record that is ordered at the same time. If no record is found or no copy is made, state law requires that we keep $12.00 for a search fee. You will receive a refund of any amount you paid over the required $12.00 search fee. The cost of a marriage or divorce coupon is $10.00 for each copy. If no record is found or no copy is made, state law requires that we keep $10.00 for a search fee. You may request a refund of any amount paid over the required $10.00 search fee. We accept cash, checks, money orders made out to Arkansas Department of Health, and and all major credit cards, including American Express®, Discover®, MasterCard® or Visa®. Please do not send cash in the mail.

Arkansas HealthCare Access Foundation serves all qualified applicants who are low-income, uninsured citizens throughout Arkansas. Arkansas Health Care Access Foundation (AHCAF) is a statewide volunteer health care program which was developed to help low-income, medically uninsured Arkansans gain access to non-emergency medical care. The program works by providing referrals to medical personnel who volunteer their goods and services. Currently volunteering their goods and services are physicians; dentists; RNs; APNs; hospitals; pharmacies; pharmaceutical companies; private home health agencies; the Arkansas Department of Public Health; and Community Health Centers. Through the continued support of the Department of Human Services, the Arkansas Department of Health, and our volunteer health care professionals, AHCAF has been able to provide non-emergency health care to thousands of Arkansans.
